How to use QR Code attendance as COVID19 contact tracing application?

When a person organizes, providing or conducting the following activities:
indoor sport, fitness activities; indoor public meetings; ceremonies; provision of personal care services; provision of public entertainment; auctions and inspections of premises for the purpose of sale or rental of any property; or driver instruction, they need to save contact tracing record. QR Code Attendance add-on could be useful.
For example, organizator will collect phone number, email address, temperature, attendance time when an attendee comes to event. Before events occurs, a secretary will create QR Code for each attendee and send all QR Codes to them. When they comes to event, she will check temperature of each person. And then she will scan the attendee’s QR Code and enter their information.
1. Create Google Sheet data

2. Set up QR Code

  • Click on QR Code Attendance/QR Code Scanner Generator menu
  • Choose UUID or Name of Attendee column for QRCode identifier. Each attendee has an unique ID
  • Enter Contact Tracing for Form Title
    2.1 Set up for Email
  • Click on Plus button
  • Enter Email for Select Label
  • Enter formula {{ INPUT }} for Select Value. {{ INPUT }} formula will require attendee to enter their email
  • Enter SET for ACTION
  • Choose Email column for To Column
    2.2 Set up for Phone
  • Click on Plus button
  • Enter Phone for Select Label
  • Enter formula {{ INPUT }} for Select Value.
  • Enter SET for ACTION
  • Choose Phone Number column for To Column
    2.3 Set up for body temperature
  • Click on Plus button
  • Enter Body degree for Select Label
  • Enter formula {{ INPUT }} C for Select Value. {{ INPUT }} C means that temperature celsius
  • Enter SET for ACTION
  • Choose Temperature column for To Column
    2.4 Set up for Attendance time
  • Click on Plus button
  • Enter CHECK IN for Select Label
  • Enter formula {{ NOW }} for Select Value. {{ NOW }} means that add-on will write current date for attendance time
  • Enter SET for ACTION
  • Choose Attendance Time column for To Column

    3 Set up QR Codel Label
  • Choose Name of Attendee for QR Code Label
  • Choose 1-Label-Strip for Mode
  • Choose High for Error Correction Level

    4. Print PDF file
  • Click on Preview to see the QR Codes in PDF format
  • Click on the print icon in the top right corner to print all QR Codes
    3 Save QR Code Images
  • Choose QR Code Image column for Save QRCode Image To
  • You can add logo to QR Code by clicking on Choose File button to choose your branded logo
  • Click on Save to Sheet button. Add-on will write all QRCode images into QR Code Image column

    4 Scan QR Code to write contact tracing data to Sheet
  • Secretary will scan QR Code for each attendee, for example, she will scan QR Code of Peggy F. Arnold. add-on will redirect to Google Form immedially
  • She will enter all information such as email, temperature and other information
  • Finally, click on Submit button. The add-on will write contact tracing data to Google Sheet file
